  • Overview

    The Shared Living Assistant will provide direct oversight/monitoring of assigned shared living placements and supervise 30hr support staff. 





    • Participate in screening process for potential providers, identifying matches and transitioning clients into the the Shared Living placements
    • Assume case management responsibilities including completing ISP assessments and progress summaries for assigned caseload, coordinating medical care, auditing finances monthly and communicating with other team members.
    • Visit and monitor shared living placements minimally once per month. Submit monthly monitoring reports to Deputy Director by the 7th of each month.
    • Ensure providers and job coaches are fully trained (CPR, First Aid, Med in-service, Human Rights, fire safety, OSHA, behavior plan, on call, etc)
    • Ensure that all DDS documentation has been arranged and set up with provider  at the home(med book and provider book which include all data tracking required for ISP and clinical)
    • Complete purchase orders monthly for client reinforcement
    • Supervise support staff (30 hours per week per client) and formulate job descriptions for each support staff and client (if working). Provider oversight to support staff including hiring, training, and performance reviews) in conjunction with the Director of Shared living.
    • Assume role as support staff ( day support) if required for coverage
    • Maintain budgets including mileage, respite and support staff hours, as well as managing client funds
    • Provide on-call support and assume role as primary on-call at scheduled times.
    • Enter behavioral data monthly
    • Maintain confidential files in accordance with the regulations of the Department of Development Services.
    • Performs other job-related projects and activities as requested.
    • Maintains consistent coommunication with funding sources and outside providers such as day services, DDS
    • Report any issues to the Shared Living Director



    Excellent knowledge of Microsoft Word, Excellent communication skills, strong telephone skills, strong people skills, professionalism and confidentiality.


    Education Required:

    • Bachelors/Associate Degree or substitute with experience in the developmental disabled population.
