    To complete delegated assignments within the group homes. This includes assessments of the individual’s wellness which includes medical, nutrition, mental health, prevention and education. The LPN educates both staff as well as the individuals in various settings. They are an important liaison within the agency in addition to funders and providers outside the agency. 


    This LPN role is a mobile position that will travel to and from all CRJ residential programs in Massachusetts, so ideal candidate must be willing and able to travel frequently (Mileage Reimbursed).


    Functional Responsibilities:

    • The employee must be able to demonstrate effective communication skills with staff and individuals within the agency as well as providers such as outside state funders, Healthcare providers, parents and guardians.  This will include phone, email, texting, Skype correspondence as well as in person dialogue. 
    • The employee is responsible for monitoring assigned programs and individuals within these programs.  This will include in home visits to complete standard head to toe Nursing assessments, required procedures as well as mental health/wellness checks.
    • The employee is responsible for educating both staff as well as the individuals we serve on various healthcare topics including agency required trainings. This is done in both group settings as well as one to one settings.
    • The employee is trained and implements “Medication Administration for License Practicing Nurses” including ongoing medication audits within all programs.
    • The employee while working autonomously must prioritize and report issue of importance in a timely manner to primary Nurse.


    • Licensed Practical Nurse Degree
    • Psychiatic experience preferred
    • Able to work Autonomously when performing Group home assignments
    • Prioritize and report acuity of situations and conditions
    • Works well in high stress, and emergency situations
    • Remain in compliance with CPR/ CEU’s
    • Honest and Dependable
    • Must be able to maintain professional boundaries with all working parties
    • At least two years’ experience working  with Adults with Developmental disabilities and or within the Mental health system is highly preferred
    • Must be certified in MAP ( in house training)
    • Must be CPR certified
    • Maintain and active LPN license at all times
    • PAC trained ( in house training)



    Technical Competencies:

    Must be knowledgeable in basic computer skills, for correspondence purposes; email, Skype, Word


    Physical Demands:


    The job requires basic standard Nursing care skills. The employee may need to assist the individual in bathing, toileting, transferring, and ambulating. You are expected to follow any body mechanics training and guidelines under your LPN license.


    Working Conditions – Working conditions include often performing in a high stress, acute environment that may include episodes of aggressive behaviors towards others or themselves. With a rotating schedule you are required to be on call 24 hours a day, weekends, and holidays. Driving to assigned programs is required in your own vehicle.
